Rule 7. If an applicant is a corporation, it shall
submit a recent annual report. If the
applicant is not a corporation, it shall submit the name of the registered
business in the form of a D.B.A. (doing business as) certificate indicating the
assumed name under which the applicant is conducting business and resubmit the
D.B.A. certificate upon making any changes to the business name or business status.
